BackgroundHealth screening is undertaken to identify individuals who are deemed at higher risk of disease for further diagnostic testing so that they may possibly benefit from interventions to modify the natural course of disease. In Singapore, screening tests are widely available in the form of a package, which bundles multiple tests in one session and commonly includes non-recommended tests. There are various ethical issues associated with such testing as they may not be clinically appropriate and can result in more harm (...) than benefit. This article describes the practice of health screening packages, identifies the ethical issues arising from such packages and discusses the implications of these ethical issues on policy and practice of screening in Singapore.MethodsA content analysis of the websites of providers offering general health screening packages to individuals was conducted. A total of 14 health screening package providers were analysed for how packages were conducted and promoted, how clinically appropriate screening tests were, and the price range and composition of screening packages. A normative ethical analysis based on the four principles approach of beneficence, non-maleficence, autonomy and justice in biomedical ethics was used.ResultsTwelve of the 14 providers included non-recommended tests such as tumour markers, treadmill stress tests and MRI scans in their general health screening packages. Package prices ranged from S$26 to S$10,561, with providers charging higher when more tests were included. Health screening packages were broadly conducted in three stages: the offer and selection of a health screening package; medical assessment and performance of screening tests; a post-screening review. While material provided by all providers was factual, there was no information on the potential risks or harms of screening.ConclusionSeveral ethical issues were identified that should be addressed with regard to health screening packages in Singapore. A key issue was the information gap between providers and patients, which may result in patients undergoing inappropriate testing that may be more harmful than beneficial. Health screening packages can stimulate unnecessary demand for healthcare and contribute to an inequitable distribution of healthcare resources. (shrink)

In this essay, I find much to admire and little to disagree with in Shu-Shan L ee ’s use of James Scott’s “public transcript” framework to excavate a theory of political obligation that applies to common people in premodern China. I offer some ways to further explore the implications of Lee’s analysis, in part by connecting Lee’s essay to related work on the obligations of elites. I then build on Lee’s own suggestions of connections to contemporary empirical attitudes and contemporary (...) normative views, asking in what ways Lee’s account of public obligation might be able to make room for the idea that the people, as political children, might one day be able to grow up. In this paper, I examine a case for fundamental indeterminacy (FI) by Elizabeth Barnes and offer my counterarguments. Barnes' account of FI includes both the characterization of FI and why we need to accept it. I argue that her reasons for accepting FI can be challenged even when we accept her characterization of FI. Her main claim is that finding a fundamental proposition that our fundamental theory is indeterminate about (FPF) gives us a reason to accept FI in metaphysics. I (...) challenge her claim by pointing out more plausible options to address FPFs. An FPF may either indicate that the theory is nonfundamental or lead us to accept the antirealist view; there is no room for FI in either option. One may insist on accepting FI, but I argue that it is not theoretically rewarding enough. Hence, Barnes' case for FI can be contested. (shrink)

In an award-winning essay, Shu-Shan L ee discusses scholarly commentary about obedience to the emperor, focusing on public and hidden records of protest. The thesis of Lee’s essay is that the relationship between authority and subject in imperial Confucianism was built on a conditional obligation of obedience, despite traditional accounts of it as absolute. On his account, the obligation of obedience should be conceived through the rubric of imperial Confucianism as being conditional on fulfillment of reciprocal obligations. As part of (...) my response, I suggest that there is a third way to understand the obligation of obedience through the Confucian lens, which is less transactional than reciprocal obligation, while also providing a plausible alternative to absolute obligation as the correct understanding of imperial Confucian thinking about obedience to the emperor. Proponents of “broad scope” theories of intention argue that agents cannot intend to achieve given ends without intending certain inevitable or probable consequences. I shall argue that some Thomistic variants of these theories collapse into the Expectation View (EV), i.e., that we intend to produce all of the consequences that we expect to result from our actions. I shall then raise four objections to EV. First, EV falsely implies that we intend to produce all of the expected beneficial consequences of (...) our actions. Second, EV falsely renders altruistic self-sacrifice a species of suicide. Third, EV falsely implies that medical interventions with probable fatalities must involve intentional killing even when, of the available options, they offer the most likely prospects of long-term survival. Fourth, EV falsely implies that foreseeably fatal actions must involve intentional killing even when they will foreseeably prolong the agent’s life before killing him. (shrink)

Abstractabstract:This study examines the origin and religious roots of taegyo, Korean traditional prenatal education, and raises concerns about potential negative impacts of contemporary taegyo practice from feminist and disability perspectives. Taegyo has been accepted without much criticism due to its deep integration into prenatal care culture, and most existing literature focuses on taegyo's positive impacts on fetal health and development from scientific or nursing perspectives. This article analyzes a 19th-century taegyo manual, Taegyo Singi, and Seon and Won Buddhist literatures on (...) taegyo in order to understand the religio-cultural concepts and contexts of taegyo. The article then discusses the potential downsides of taegyo practice today, considering its patriarchal, mother-blaming, ablest roots in Korean history and culture. The author raises concerns about social oppression, the control of women's bodily autonomy, and the disproportionate responsibility burden that taegyo places on Korean women. The article concludes with suggestions for future research and for well-balanced taegyo practice. (shrink)

Rappaport's comment includes several errors. First, he conflates manipulation and deceit. Second, he confuses the rationalism of the evolutionary biological analysis of organisms with the rationalism (or lack thereof) of the motivational and cognitive structures of the organisms under study. Third, his moralistic judgment of my focus on manipulation implies that scientists should not only not explore but should also suppress such unsettling ideas. We will make little progress in understanding morality and in fostering truly moral behavior if we refuse (...) to acknowledge that moralistic statements may sometimes, and perhaps even often, be used in a manipulative way. An ethics of bewilderment, which differs dramatically from the more familiar ethics of ease, is best understood through poetic presentations. Using examples drawn from Chinese and Western sources—notably Du Fu and Dante—this inquiry treats bewilderment as both an emotion and a virtue. Both these forms of bewilderment involve an acknowledgment of how minimal is the ethical confidence we have, given the feelings we have and the judgments we must make, but they also extend in productive ways the implications of that (...) acknowledgment. Robert C. Scharff has written what we might call, after Nietzsche, a timely meditation. It is timely in that it is aimed at our particular time , and it is a meditation on timeliness, on what it means to do philosophy within time and history . These two topics meet in his depiction of our time as one that is either not fully aware of or that actively suppresses its own timeliness, its own determination by its time and historical context, (...) due largely to analytic philosophy’s remaining caught in the vestigial grips of positivism and the dominance of science. We may be “After Positivism,” but we are not wholly past positivism. In spring, summer and autumn 2020, one abiding argument against controlled human infection studies of SARS-CoV-2 vaccines has been their impact on local communities. Leading scientists and bioethicists expressed concern about undue usage of local residents’ direly needed scarce resources at a time of great need and even about their unintended infection. They recommended either avoiding CHI trials or engaging local communities before conducting any CHIs. Similar recommendations were not made for the alternative—standard phase III field trials of these same (...) vaccines. We argue that the health effects of CHI studies on local residents not participating in the study tend to be smaller and more positive than those of field trials. That is all the more so now that tested vaccines are being rolled out. Whether or not local community engagement is necessary for urgent vaccine studies in the pandemic, the case for its engagement is stronger prior to field trials than prior to CHI studies. (shrink)

This paper argues that there is an inconsistency between Jaegwon Kim's earlier work on supervenience and his more recent work on explanatory exclusion. In his work on supervenience Kim advocates an explanatory agnosticism that, by the time of his later work, is replaced by an endorsement of reductive explanation. ABSTRACTIn the Timaeus, Plato explicitly defines time as “the moving image of eternity”. This proposition affirms actually that time reflects the eternal that embodies the rational and lawful principle – namely the logos of proportionality – in the motion and change of visible objects in the universe. In other words,time determines the principle that every mutable being must follow to participate in the rational and nomological order of existence; the absolute logos which is given by God is hence intrinsic to (...) time. This idea of time is echoed by the theories of Augustine and Husserl. Both philosophers inherited some of Plato’s metaphysical views, and each of them developed a distinctive theory of the relations between Being, time, and the absolute logos. In 2011 the Journal of Moral Education (JME) celebrated its 40th anniversary of publication. It seemed appropriate to examine and reflect on the JME?s achievements by reviewing its evolution and contribution to the emerging field of moral education and development. Moral education trends, as reflected in the 945 articles published in JME from 1971 to 2011, were investigated by content analysis. The research objectives were: to discover the trends in moral education as represented by published articles and special issues (by (...) analysis of disciplinary approaches, key topics, research methodologies and age-related educational levels) and to examine the international and gender-related development of the journal and the influence of its contributors (by analysis of first authors and editorial board members). The findings identify important concerns, key research topics and neglected areas in moral education and development. Analysis offers an insight into the contribution of JME as the main international interdisciplinary journal in this field, to the history of moral education, to theory and practice and in the changing socio-cultural contexts of the past 40 years. Discussion of the findings is offered, limitations are acknowledged and implications for future directions for the journal considered. (shrink)

ABSTRACTThis study examines whether a firm's ownership form has any influence on its social performance. Conventional wisdom suggests that public corporations are more susceptible to corruption and socially irresponsible behavior than privately owned corporations because of the intense short‐term profit maximization pressure from shareholders and the lack of sufficient monitoring mechanisms. This study introduces an alternate perspective in thinking about the relationship between ownership form and corporate social responsibility. This study reasons that public corporations are more likely to become socially (...) responsible because of their greater exposure to external influence and greater dependence on external actors with diverse interests. Using a panel data on the pollution management practices of 118 industrial facilities over a 13‐year period, this study shows that public corporations were indeed subject to greater external pressure to reduce pollution, and the pressure led to consistently stronger environmental performance. What assumptions are built into the claim that experience has “phenomenal properties,” and could these assumptions turn out to be false? I consider the issue specifically for the similarity relations between experiences: for example, experiences of different shades of red are more similar to each other than an experience of red and an experience of green. It is commonly thought that we have a special kind of epistemic access to experience that is more secure than our access to the external (...) environment. In the first part of the paper, I argue than one way of elucidating this claim is especially plausible—that systematic error, of the kind subjects make about the external environment in traditional “skeptical” scenarios, is not conceivable for introspection of experience, including for our knowledge of similarity relations. I argue that focusing on similarity relations gives us a more interesting version of the argument than for other forms of experiential introspection. Then in the second part of the paper I describe an example, inspired by a similar case due to Sydney Shoemaker, in which a subject, despite being fully rational and attentive, apparently is systematically mistaken about the character of their experience in a surprising way. I argue that the example calls into question whether there are properties of experience satisfying the epistemic access constraint, and therefore whether experience has “phenomenal properties” in the intuitive sense. This paper propounds a new reading of Berkeley's account of the activity of finite spirits. Against existing interpretations, the paper argues that Berkeley does not hold that we causally contribute to the movement of our bodies. In contrast, our volitions to move our bodies are but occasions for God to cause their movement. In answer to the question of wherein then consists our activity, the paper proposes that our activity consists in the dual powers to produce (1) our volitions ? This article seeks to model the agenda-setting strategies of stakeholders equipped with online and other media in three cases involving protests against multinational corporations (MNCs). Our theoretical objective is to widen agenda-setting theory to a dynamic and nonlinear networked stakeholder context, in which stakeholder-controlled media assume part of the role previously ascribed to mainstream media (MSM). We suggest system dynamics (SD) methodology as a tool to analyse complex stakeholder interactions and the effects of their agendas on other stakeholders. We find (...) that largely similar dynamics of interactions occur among stakeholders in these cases, and that the costs for managements of maintaining their agendas steadily rises. We conclude that the “web of watchdogs” comprises a powerful reason for managers to engage in responsibility negotiations with their stakeholders. (shrink)

The ‘byproduct account’ of female orgasm, a subject of renewed debate since Lloyd (The case of the female orgasm, Harvard University Press, Cambridge, 2005), is universally attributed to Symons (The evolution of human sexuality, Oxford University Press, Oxford, 1979). While this is correct to the extent that he linked it to the adaptive value of male orgasm, I argue that the attribution of the theory as we understand it to Symons is based on a serious and hitherto unrecognised misinterpretation. Symons (...) had a different explanation of why women can orgasm, and beneath this explanation lies an obscure line of argument, including a particularly obscure use of the word ‘homologous’. In his 2004 paper ‘Tensed Quantifiers’, David Lewis presented an apparently powerful case for eternalism by arguing that we cannot account for the truth-conditions of sentences like ‘There have been forty-four presidents of the United States’ and ‘There will be five more presidents of the United states’ and maintain a non-revisionary attitude towards their truth-values, without committing to the existence of ‘past’ and ‘future’ things.
